Your first visit is an important step toward relieving your dental pain and saving your natural tooth. We want to make this experience as comfortable and efficient as possible.
What to Bring
- Insurance Card & Photo ID
- Referral Form from your dentist
- Recent X-rays (if applicable)
- List of current medications
Important Info
- • Please arrive 15 minutes early to complete paperwork.
- • Inform us of any allergies or medical conditions.
- • Eat a normal meal before your appointment unless instructed otherwise.
- • Most appointments last between 90 minutes to 2 hours.
